4. Appointment Scheduling

Submitting a request through our online scheduling form or contact form does not guarantee an appointment. All appointments are subject to confirmation by our office. We will contact you to confirm your appointment date and time.

5. Wellness Plans

Cafe Smile Studio offers Prepaid Care Packages ("Wellness Plans") for patients without insurance. Wellness Plans are prepaid bundles of specific dental services at a set price. They are not insurance, not a dental discount plan, and not a Dental Maintenance Plan Organization (DMPO). Full terms of each Wellness Plan are provided in the Wellness Plan Agreement at the time of enrollment. In the event of any conflict between information on this Site and the Wellness Plan Agreement, the Agreement governs.

6. Pricing and Payment

Treatment pricing displayed on this Site, if any, is approximate and provided for general reference only. Actual costs depend on individual clinical needs and are determined during your consultation. We accept most major dental insurance plans, and our office will assist with verifying your benefits. Payment is due at the time of service unless other arrangements have been made in advance.

7. Insurance

While we accept many insurance plans, coverage and reimbursement amounts are determined by your insurance provider, not by Cafe Smile Studio. We will assist with filing claims as a courtesy, but the patient is ultimately responsible for any balance not covered by insurance.

8. Intellectual Property

All content on this Site, including text, graphics, logos, images, and software, is the property of Cafe Smile Studio or its content suppliers and is protected by United States copyright and trademark laws. You may not reproduce, distribute, modify, or create derivative works from any content without our prior written consent.

Patient testimonials are not guarantees of outcomes. Before-and-after images are of actual patients treated at Cafe Smile Studio and are displayed with patient consent. Arbitration shall take place in Denton County, Texas. This arbitration provision does not apply to claims for dental or medical malpractice, which are governed by the Texas Medical Liability Act and applicable state law. Either party may bring qualifying claims in small claims court. Cafe Smile Studio will pay all arbitration fees and costs beyond the initial consumer filing fee.

You may opt out of this arbitration provision by sending written notice to Cafe Smile Studio at 2330 Justin Rd, Suite 300, Highland Village, TX 75077 within 30 days of first using this Site.

16. Governing Law

These Terms shall be governed by and construed in accordance with the laws of the State of Texas, without regard to conflict of law principles. Any legal proceedings not subject to arbitration shall be brought in the courts of Denton County, Texas.

You agree to indemnify, defend, and hold harmless Cafe Smile Studio, its owners, employees, and agents from any claims, damages, losses, or expenses (including reasonable attorney fees) arising from your use of the Site or violation of these Terms.
